THE SOLUTION: Turn the phone OFF and then turn it back ON. Go to Settings app and find Assistive Touch, then manually turn it ON and OFF repeatedly (I usually do it 10-15 times). While I'm doing that, when the slider is in the ON position, sometimes the Assistive Touch icon appears instantly, sometimes not.
